The New Member Process:

The prospective member will be interveiwed by the Personnel Services Officer or designee who will complete the Prospective Member Interview Worksheet (CGAUX-2) and assist the prospective member in completing the New Member Application (CG-32).  Upon completion of study of the New Member Reference Guide, the prospective member shall complete an open-book examination and record his/her answers on the New Member Examination Answer Sheet.

New members must submit the following completed forms:

red button New Member Application (ANSC7001)
(Or obtained from the Personnel Services (PS) Officer).
cerise button Prospective Member Interview Worksheet (ANSC7036)
(
Obtained from and completed by the Personnel Services Officer).
purple button New Member Examination Answer Sheet
(Obtained from the PS Officer.  The examination is administered and graded by the PS Officer).
blue button Two (2) completed Fingerprint Cards (FD-2858)
(Forms obtained from the PS Officer and are specific to the Coast Guard...do not use other agencies' forms).
sky blue button Authorization for Release of Information (SF85)
(Two copies, each with original signatures).
turquoise button Special Agreement Check (SAC) Form (OFI-86C)
(Two copies, each with original signatures). 


Upon submission of these forms, an Employee ID Number (EMPLID#)  will be issued. The member is placed  in the "Approval Pending" flotilla.  Upon passing the security check, the new member will be transferred to the flotilla of his/her request and a CG Auxiliary Identification Card issued upon receipt of the member's identifying information.
